Case Studies

Queensland Rail

A statutory rail authority providing rail services wanted to improve the approach, objectivity, repeatability and robustness to safety risk decision-making. Through a Forge Works partnership a formal process and accompanying tool for the evaluation and selection of reasonable risk reduction options was customized and integrated within existing change and risk management processes to provide a repeatable, objective and easy-to-use method that improves decision-making and demonstration that safety risk has been reduced so far as is reasonably practicable.

Fortescue Metals Group

A mining sector rail operator wanted to ensure that all reasonably practicable controls have been provided to manage rail operations in ‘dark territory’. Through a Forge Works partnership, incident investigation findings, major hazard control audit findings, operating procedures and risk assessments were reviewed and a workshop was conducted to ensure that all legitimate threats to safe operations, existing controls and associated assurance activities are documented in the bow ties, and any additional practicable controls are identified and assessed for reasonability using a customized, repeatable, objective and easy-to-use SFAIRP demonstration method.

Hewitt Cattle

A family owned and operated cattle and farming company wanted to ensure that they understood and were able to manage their critical risks following a significant incident. Through a Forge Works partnership an industry leading critical risk process was embedded within their existing risk management system, and through real application, internal HSE advisors were mentored to a level of capability that enabled them to implement and sustain the new process.

Powerco

Marand Precision

A global supplier of precision engineered solutions to a range of industries including aerospace, defence, rail, automotive, mining and energy, wanted to simplify their plant risk management process to better enable their limited resources to control the various hazards associated with their high risk plant. Through a Forge Works partnership, our consultants liaised with the operational personnel who conduct the work, reviewed their current assessments and registers, with consideration to local legislation. Forge Works were able to cut through the system clutter to design a new operational tool which enabled the organization’s limited resources to focus and prioritize their improvement efforts towards items of plant that presented the greatest level of risk.
